Payment Policy
Payment must be made at time of service or program enrollment unless other arrangements have been made in advance.
Synergy Therapies will assist in obtaining insurance coverage information. The patient or family is, however, responsible for knowing their insurance policy.

- Insurance Assignment including various HMO's & PPO's
- Fee for Service
- Cash, check, Visa, Mastercard, Discover & American Express accepted

- Blue Cross/Blue Shield - All Products (PPO, HMO, Blue Care, Blue Advantage etc.
- Humana - All Products (HMO needs referral generated from physician)

- Work Comp - (Aetna, Focus, CorVel, any other with Case Manager Approval)
- Coventry - All Products (PPO, HMO, Advantra, First Health, CCN)

- PHCS (No Golden Rule, American Medical or Sprint Select plans)
